POLICY All verbal orders shall be documented by person receiving orders in patient’s medical file and properly write with date, time, name and signature of the person making entry . These documented orders shall be counter signed as soon as possible (within 24 hours) by the consultant who has given the orders.

POLICY The cross check shall be endorsed by sign, name, date and time of the consultant. Avoid use of abbreviations while taking verbal orders.

Policy Verbal order shall not be given for high risk medicines unless emergency . Verbal orders follow only in emergency situation . To follow same protocol for critical test reports

Verbal order REPEAT IT BACK During code the nurse who giving medicine should announce eg .: Adrenaline 1 amp.IV given .

Medicine can give verbal order ● All emergency medication using in cardiac arrest ●Medication using for convulsion ●Muscle relaxant during intubation

Verbal Orders must not be given for:  Controlled Drugs via any route (excludes Clinical Emergencies where the prescriber is present .)  Epidural boluses - unless authorised Acute Pain Management Service (APMS) Nursing Staff

Verbal Orders must not be given for:  Blood/blood products (verbal orders may occur in operation theatre or Emergency department under emergency situations e.g. the Massive Transfusion  Children/Neonates

Verbal Orders must not be given for:  Patients with documented history of chronic kidney disease or acute renal failure  Abortion inducing medications  Labour inducing medications
