Topiramate is a medicine used to treat epilepsy and to prevent migraines. Common brand names : Topamax, topirol , topaz. Drug class : Sodium channel modulators. Topiramate, a medication used to treat epilepsy and migraines, can have significant effects on the eyes, particularly in relation to glaucoma. It has been associated with causing or worsening a specific type of glaucoma known as AACG(acute angle closure glaucoma). Topamax comes as 25mg,50mg,100mg,200mg.
MECHANISM : Topiramate can cause ciliochoroidal effusion (collection of fluid in hollow spaces), which leads to the swelling of the ciliary body. This swelling can push the lens and iris forward narrowing the angle between the iris and the cornea and potentially blocking the drainage of aqueous humour. This blockage can result in sudden increase in IOP, leading to AACG. Topamax comes as 25mg,50mg,100mg,200mg.
SIDE EFFECTS : Not feeling hungry, weight loss, tingling of the arms and legs(paresthesia), speech problems, fatigue, nervousness, abnormal vision, suicidal thoughts, eye problems including acute myopia(nearsightedness), and secondary closed angle glaucoma, visual field defects and vision loss.
POINTS TO BE NOTED: If you are pregnant you should speak to doctor before taking topamax , it can harm to the foetus. If you are taking topiramate and experience symptoms such as sudden eye pain, headache, blurred vision, seeing halos, around lights or nausea, it is crucial to seek immediate medical attention. Other issues may include retinal complications such as uveitis, visual field defects, myokymia, neuro ophthalmic complications, increased corneal thickness and possibly scleritis.
CILIOCHOROIDAL EFFUSION SYNDROME A ciliochoroidal effusion is an abnormal accumulation of fluid in the suprachoroidal space(space between the sclera and choroid) of the eye that can cause a forward rotation of the ciliary body and forward movement of the iris-lens diaphragm (LID). In some cases, this can lead to a transient myopic shift in vision or even blockage of the eye internal drainage system, leading to secondary to AACG. The major complication of ciliochoroidal effusion syndrome is secondary angle closure glaucoma.
Topamax isn’t approved to treat depression or bipolar disorder with depression, but it may be helpful for people who have not found relief with other medications. Some people used Topamax to treat other conditions like anxiety, depression, bipolar disorder or post traumatic stress disorder(PTSD), but Topamax is not approved by the Food and Drug Administration(FDA) for these purposes. Topamax is a prescription drug that belongs to a class of drugs called anti- convulsant or antiepileptic drugs(AEDs).
