Objectives General Objectives: At the end of the session the participant would be able to explain about under five clinic and well baby clinic.

Specific Objectives At the end of this session the participant will be able to: Define well baby clinic Activities of well baby clinic Component of well baby clinic Health check –up and timely assesment of under five children.

Introduction Under Five Clinic is a center where care is provided in a package manner to under five children or preschool children for comprehensive health care under one roof . It is not a separate institution. This center is also known as well baby clinic.

Introduction (1/3) Therefore, these services are economical for a large numbers of young children . I t also emphasizes early identification of developmental delay in young children below 2 years of age.

Reasons for Under Five Clinic Under five age groups are vulnerable and special risk group . Under 5 occupy a large sector of total population with high morbidity and high mortality . They are preventable with adequate care through immunization, sound nutrition, health education and environmental sanitation. This age group also needs regular monitoring for growth and development.  

Objectives To provide comprehensive health care to young children in a specialized facility . To prevent childhood problems such as malnutrition,pertusis,tuberculosis,poliomyelitis , diphtheria, tetanus and measles.

Objectives (1/2) To provide simple treatment for diarrhea, with or without dehydration, pneumonia, skin conditions and other common disorders. To provide supervision of the health of all children upon the age of five. To promote the education of parents in order to promote health and family planning.

Main Components of Under 5 Clinic Care in illness or curative services It is the apex of the symbol and the basic philosophy of the clinic. It is the felt need of mother. Seventy to ninety percent of illnesses can be treated with trained nurses . Nurse can play an important role in taking care of sick children.

Main Components( Contd …) Treatment of common childhood illness such as diarrheal diseases, worm infestation, acute respiratory infections, skin infections, scabies, malnutrition etc.

Main Components ( Contd …) The nurse learns to differentiate between minor common illness and serious illness which need referral for special care. The curative services include:   Early diagnosis and treatment of acute and chronic illnesses X-ray and laboratory services Referral services

Preventive care Immunization Immunization services are provided by under-five clinics to prevent the child from contacting serious illness. Children can be protected by a fairly cheap and simple program on immunization.

Immunuzation ( Contd …) Prevention is always better than cure . It is more cost effective to protect people from a disease by immunization than to treat them when they get the illness.

Growth Monitoring Growth monitoring is the one of the basic activity of under 5 clinic. It is the best available indicator of the overall nutritional status of the child . It is done by weighing the child periodically at monthly intervals during the first year, every 2 months during the second year and every 3 months thereafter up to the age of 5 to 6 years. "

Growth ( Contd …) The child's weight is plotted on "road to health card as "growth curve” which helps to detect early onset of growth failure

Growth ( Contd …) Growth chart is the simplest, inexpensive, effective tool for monitoring the child's health and nutritional status. The common causes of growth failure are failure to B reastfeed, I nadequate nutrition , I ntestinal parasites , A nd infection.

Growth ( Contd …) It helps to identify the child at risk so special care and and timely referral can be done . Nutritional surveillance - Illness is closely related to malnutrition. Common nutritional disorders (PEM, anemia, and rickets) occur in this age group .

A poorly nourished child has a low resistance to disease . A young child who does not get enough good food will have impaired physical and mental growth

Growth ( Contd …) A sick child needs special nutrition and extra protein foods to build up his strength again . Health education regarding adequate nutrition is provided in the under five clinic.   Supplementary foods started at the age of 6 months when the iron stores in the liver are depleted so the infant needs a diet rich in iron.

Growth ( Contd …) Continue breast feeding until the end of the second year of life, together with supplements.   Ensure cleanliness and hygiene during food preparation .  

Growth ( Contd …) Family planning services It is possible to conduct family planning program through the clinics.   The mother is counseled on the various options available, their merits and demerits so that they can make choice.  

Health checkup and timely assessment The health checkup cover appropriate laboratory tests, physical examination and The child health card provides a check list for these examinations.

Oral rehydration therapy The mothers are taught oral rehydration therapy in case the child has diarrhea. Environmental sanitation should be maintained for child welfare.

Health education It is one of the essential activities of under 5 clinic. The clinic used to educate the mother about child caring and rearing practice B reast feeding , N utrition, G rowth monitoring, immunization, and cleanliness.

Health Education( Contd …) It also includes preventive measures against infectious diseases , parasitic infestation etc . when she visits under 5 clinic with her baby.
