A clinical pharmacist is responsible to interact with the patient on multiple occasions from the patient admission in the ward to the end of treatment and even beyond that. During these interactions, the initial days are reserved for gathering information about the patient, the later stages involve informing the patients about their post-hospital routines. Generally, the “information gathering” is known as an interview and “information giving” is termed as counselling. These processes may interchange in some situations where the information is given during the interview and gathered in the counselling sessions. What is the Difference Between Interview and Counselling:

What is Patient Medication History Interview? Patient medication history interview (PMHI) is a practice associated with collection and recording of information by re-viewing or re-interviewing of patient related to his/her past and present medications used. Clinical pharmacists are responsible for dealing with such type of activity. Clinical pharmacists collects detailed, accurate and complete information of all prescribed and non-prescribed medications which patient have taken previously or currently under use .

Prescription errors can be prevented and consequent risk to patients can be avoided. Helpful in finding of drug related pathology and any clinical signs due to results of drug therapy. A full medication history identifies patient’s needs, explores the patient’s perspective of illness and its treatment. All accurate medication history information (such as currently and recently prescribed drugs, previous ADRs including hypersensitivity reactions, any OTC medications including herbal or alternative medicines, etc.) helps to establish better health care plan for the patients Importance of Accurate Medication History:

Helps to investigate medication discrepancies (variations) by comparing the medication profile with the medication administration record. To verify medication history taken by other staffs and provide additional information where appropriate. Helps to document the allergies and drug related ADRs. Helps in screening of drug interactions. Helps to assess indication/evidence of drug abuse. Helps to evaluate the rationale for prescribing of drug. Helps to evaluate drug administration techniques. To provide better patient compliance (acceptance). Need for Patient Medication History Interview:

Purpose: To provide a consistent method to obtaining the essential components of a complete medication history. Important points that are to be discussed with the patient that are to be discussed with the patient are underlined and are to be covered with patients. Medication Interview Forms

Demographic Data: Patients name: Male/Female: Consultant: Ward: Date of hospital admission: Date of Interview: 1. Prescribed medication: (what medicines are you having at the moment?) Record what the patient says and make a note of anomalies with their correct prescriptional drugs Previously any drug has been prescribed (with date/duration if possible)? 2. Non-prescribed medication: Did you take any OTC drugs or any health consumables items from a shop without a prescription-chemist, health food stores, super market? Currently being used. Used previously (with dates/duration if possible) 3. Social drugs : If yes, ask patient how much they are taking per week/day? a. Smoking b. Alcohol c. Illicit drugs 4. Response to drug therapy: Whether the current medication is benefiting you? If yes, how? If no, why? Do you think your previous medication benefited you? If yes, which ones and how? If no, why? 5. Do any of the things you buy without a prescription, helped you? If yes, how much?

6. Side effects: Are you suffering from any side effects now? If yes, what side effects. Which of your medicines do you think is causing the problems? Have you suffered any side effects with previous drug treatments? If yes, what side effects and with which medicine? 7. Compliance: Do you take any assistance for administration of your medication? Did you miss any dose? When? How long? Why? 8. What medicines regularly and usually take for: a. Headache b. Aches/Pains and flu c. Allergy 9. To whom you ask to take medicines for headache, allergy, flu, etc. a. pharmacist/chemist b. relative/friend c. nurse d. doctor e. no one 10. Any other problem with drug therapy? 11. Any comments: 12.
