Urinary catheterization : Urinary catheterization is placing a tube in the body to drain and collect urine from the bladder INDICATIONS 1 .Urinary obstructions 2.Urinary trauma 3.Voiding disorders 4.Urine diversion during surgery 5.Collection of urine samples for the diagnostic purpose
Types of urinary catheterization Intermittent catheterization : urinary catheters are passed and removed to obtain urine samples to relieve an anatomical or functional obstruction Indwelling catheters: urinary catheters are left in place for variable periods Indicated for urinary retention and obstruction due to neurogenic bladder dysfunction
general preparations and Positioning Clip the hair from the vulva and distal end of the prepuce to maintain the hair free area around the catheter insertion Cleanse the area using antiseptic solution Flush prepuce or vulva by using 0.05% chlorhexidine solution 4-5 times Sedation is done if needed Lateral recumbency or dorsal recumbency or standing is achieved based on the requirement
Urinary catheterization – dogs and cats Male Extrude the penis from the prepuce and keep extended and measure the length of catheter(foleys catheter) needed by measuring tip of penis to pelvic brim Lubricate the tip of catheter with sterile jelly and slide the catheter through penile urethra until urine leaks from end of catheter Female Blind method : estimate the length of catheter( pelvic brim to lips of vulva)
Spread the vulval lips and catheter is passed into urethral orifice ,resistance is met it enters vagina Urine appears if it reaches urinary bladder Visual method : 1. using speculum- vaginal speculum is inserted in inverted position to visualize the orifice 2.using otoscope-insert the catheter on the ventral floor of otoscope directly into the urethral orifice Digital method : guide the catheter using finger by palpating the vaginal vault Introduce the catheter into urethral orifice
Dog catheterization
Urinary catheterization-horse Male (stallion) Bladder catheterization in male horse is accomplished using a male equine urinary catheter(stallion catheter) Glans penis should be cleaned with the chlorhexidine scrub and sterile irrigation solution Catheter is handled using sterile technique Lubricant may facilitate the catheter passage
Female (mare) Clean the perineum with chlorhexidine scrub and sterile irrigation solution A sterile gloved hand should be placed into the vaginal vault and the index finger is advanced into the opening of urethra ( located on the floor of vaginal vault) The tip of the catheter should pass under the index finger and the catheter must advance through the urethral sphincter into the bladder until urine flows

Female Prepare the tail head of the animal for aseptic injection of a local anesthesia. Administer four to 6cc of a 2% solution of Lidocaine as an epidural The vulva and perineum is washed three times with Betadine surgical scrub A sterile plastic palpation sleeve is placed on the hand of the operator
The hand of the operator is introduced into the vestibule and the urinary opening identified and the index finger inserted into the urethral meatus to deflect the sub urethral diverticulum.
A sterile catheter of appropriate size is introduced into the urethra over the top of the sub urethral diverticulum and the inflation bulb of the catheter is filled with the appropriate volume of sterile saline.
