Well Family takes the following into consideration for collaborative actions: * Common Intake * What are Family Dynamics Over Time * How to Manage Single & Multiple Program Options * Structured Intake Path Options * Common Family Support Plan * Collaboration Rules * Identifying, Validating and Managing Participation * Explore Multiple Dimensions * Uniform Consent Process
You then have the ability to access one or multiple programs demographics and/or actionable items:
Let’s walk through an intake path: Who are you? Have you been in WFS before? Why are you here today? I am a 24 year old married female with 2 children. Yes, I was in an ICC program in 2008. I am pregnant Prenatal Intake & Case Management Well Family System directs the participation path based on Intake Type.
Here are a few examples of where a consumer will enter the process: Women’s Health Questionnaire Case Management Prenatal Intake Prenatal Assessment Case Management Women’s Health Questionnaire Case Management Case Management Infant Screen Infant ASsessment Case Management
Then a Plan of Care is initiated based upon the input data: PHYSICAL & MENTAL HEALTH Early Education & Development Social-Emotional Development Spiritual Foundation Safe & Nurturing Environment Economic Stability Info at Intake is analyzed and triggers needs in several categories. Which automatically builds a basic FSP Physical and Mental Health Dental Issues Refer to Dental Services No Infant Care Enroll with Pediatrician CATEGORY ITEM ACTION Safe & Nurturing Environment No Permanent Home Housing Assistance Economic Stability Father Not Working Refer to WIC Job Training A Plan of Care is initiated and The Family Support Plan provides a starting point for determining a path of care and can be shared across programs and can be part of the family history over time.
You can then access the individual from multiple agencies to view all services: MANAGE SHARED SERVICES WFS allows collaboration of services and information between multiple programs. The most common example is shared services or assessments. More than one agency may provide parenting education, etc. or administer an ASQ. WFS provides common services to be viewed by multiple agencies to help reduce duplication of services. Further rules can be established that determine which agency can act on the required service.
…and also view family connections. MANAGE FAMILY CONNECTIONS Where permissions are granted to single or multiple programs the WFS Family Connection posts a notice when a family member, other than the one being served in a specific program, is opened as a case in another program. A typical instance may be where there is a prenatal woman in one program and the father of the baby joins a male participation program. Over time a typical example may be an older woman, once a teen mom, in an interconceptional care program where her daughter entered Healthy Start as a prenatal intake, etc.
