Types of Wound Healing

Primary Intention - Edges approximated and closed surgically - Indications: clean wounds, minimal tissue loss - Process: minimal inflammation, little granulation, quick epithelialization - Outcome: fast healing, fine linear scar - Examples: surgical incisions, sutured lacerations

Secondary Intention - Wound left open to heal naturally - Indications: large tissue loss, contaminated/infected wounds - Process: prolonged inflammation, abundant granulation, wound contraction, slow epithelialization - Outcome: slow healing, large irregular scar, risk of contracture - Examples: pressure sores, burns, traumatic wounds

Tertiary Intention (Delayed Primary Closure) - Wound left open initially, later closed surgically - Indications: contaminated wounds, high infection risk - Process: debridement, drainage, granulation, then closure - Outcome: better than secondary, slower than primary - Examples: traumatic abdominal wounds, contaminated lacerations

Clinical Relevance - Factors delaying healing: • Infection • Poor blood supply • Diabetes mellitus • Malnutrition • Corticosteroids • Repeated trauma - Complications: • Hypertrophic scars (raised, within wound edges) • Keloids (excessive, beyond wound edges) • Chronic non-healing wounds
