ACTION PLAN FOR LEARNERS WITH SPECIAL NEEDS 2024 - 2025 Program Component Objectives Strategies/Activities Physical Target Persons Involved Time Frame Funding Requirements Success Indicator 1. Advocacy To inform the school/community about the recent development in teaching Learners with Special Needs (LSEN). Orient SPED parents inclusive education program. Present implementation Plan for inclusion in the S I P. Share the process to upgrade current SPED program At least 70% of the community should be informed about the program. -District Supervisor -Principal -PWD -SPED Teacher -PTA -Community -Teacher in Regular Class/Receiving Teacher -pupils August - November 2025 At least 80% of the school/community have been informed about the program.
2. Assessment -Orient the parents and pupils last section about the Learner and nominate, screen pupils who may need special attention. -conduct Pre-referral and Diagnostic Assessment Tools. Discussion of the implementation plan w/the school head. Implementation of Assessment Program and Individualized Education Plan Administration of MFAT Learners with Special needs must be identified in Regular Class from Kinder, Grade I up to Grade VI (Inclusive Education) -Principal -SPED Teacher -Regular Teacher or Receiving Teacher August – November 2025 3rd week of September At least 80% of the school/community with LSEN identified. Pupils Development To develop pupils with Learners with Special Needs (LSEN). An effective instruction at a curricular level that is appropriate to their current skills and to remediate their achievement deficiencies. Enrol children with Special Needs in SPED. Provide appropriate placement for Learners with Special Needs Kinder, Grade I to Grade VI pupils with the help of the Class Advisers. Learners with Special Needs were properly evaluated and placed in their proper placement. -Principal -SPED teacher -Receiving teacher -Occupational therapist SPED Fund MOOE LSEN pupils properly assessed by Assessment Team.

Instructional Materials Development Prepare instructional Materials suited for student with learning disability. Submit purchase order of instructional materials, equipment and cleaning materials. Additional instructional materials out of recycled and low-cost object. Make tactile materials. -SPED Teacher -Pupils (LSEN) -Parents -Receiving teacher -PWD, teacher Year Round SPED Fund MOOE Instructional Materials have been prepared and procured.

Staff Development To upgrade content knowledge and practices learned in training program. Inform or orient all school staff about the Transition program of SPED. Attend Seminar Enrol another unit for M.A. Principal, regular teachers and other school staff should be oriented. -Principal -School Staff -SPED Teacher August 2022 to May 2025 MOOE 90% of the staff has been oriented about the TRANSITION Program.

Tagging of Pupils in the LIS National Disability Prevention & Rehabilitation Week/ Naga City Disability and Rehabilitation Month (Advocacy Campaign i.e Hanging of Streamers) Sight Saving Month/ Sight Conservation Week National Mental Health Week National Week for the Gifted and talented International Day of Person with Disability National Autism Consciousness Week National Children with Intellectual Disabilities Week Organize SPED class for Learners with Special Needs Enrolled Learners with Special Needs must be tagged in LIS. Join the activity in every advocacy. -District Supervisor -Principal -SPED Teacher -PWD Personnel -Pupils -Parents Year Round 3rd Week of July / Whole month of July Whole month of August/1 st week of August 4 th week of Nov Dec 3 3 rd Week of Jan Feb 14-20 LSEN enrolled in the TRANSITION Program and INCLUSIVE Education Submit the report promptly.
