2. BLADDER CATHETERISATION DEFINITION Insertion of Foley’s catheter into the urinary bladder through urethra INDICATIONS 1. Acute retention of urine in BOO 2. To monitor urine output 3. Neurogenic bladder 4. Urinary incontinence 5. Instilling dye into bladder to take Cystogram or Micturating Cysto Urethrogram- MCU 6. Instilling topical chemotherapy in Ca Bladder

3. BLADDER CATHETERISATION CONTRAINDICATIONS ABSOLUTE Urethral Trauma- Rupture Urethral Stricture CONTRAINDICATIONS RELATIVE Acute Urinary Tract Infection- UTI Patients on anticoagulants/ Bleeding Diathesis

5. BLADDER CATHETERISATION SEQUENTIAL STEPS 1.Introduction & Informed consent Introduce yourself to the patient Tell the patient why and what you are going to do? Tell the possible complications also and then obtain the informed consent. 2. Chaperone & Privacy Ask for a chaperone Maintain privacy by putting the curtain

6. BLADDER CATHETERISATION SEQUENTIAL STEPS 3. Wear PPD, wash hands and wear glove PPD/PPE: Wear apron, cap, mask and goggles Wash hands Wear sterile glove

7. BLADDER CATHETERISATION SEQUENTIAL STEPS 4. Patient’s position Place the patient in the supine position with legs extended and flat on the bed. 5. Put absorbent pad under patient Place absorbent pad under patient

8. BLADDER CATHETERISATION SEQUENTIAL STEPS 6. Clean genitalia properly Retract the prepuce in un circumcised patients and clean glans with Povidine - iodine or Chlorhexidine 7. Apply sterile drape Drape patient with sterile fenestrated towel

9. BLADDER CATHETERISATION SEQUENTIAL STEPS 8. Inject lignocaine jelly into urethra Inject 10ml lignocaine jelly into bladder 9. Wait for 5 minutes Compress urethral meatus for 5 minutes to avoid back flow

10. BLADDER CATHETERISATION SEQUENTIAL STEPS 10. Place sterile receiver Keep sterile kidney dish below the genitalia 11. Select appropriate catheter Select the correct catheter for your patient

11. BLADDER CATHETERISATION SEQUENTIAL STEPS 12. Insert catheter gently into urethra Insert gently Foley’s catheter into the urethra 13.Fully insert the catheter until urine drains

12. BLADDER CATHETERISATION SEQUENTIAL STEPS 14. Inflate balloon with sterile water Inflate balloon with 30ml of sterile water 15.Attach Urobag Attach appropriate size urobag

13. BLADDER CATHETERISATION SEQUENTIAL STEPS 16. Pull catheter to conform self-retaining status 17. Replace prepuce if appropriate

14. BLADDER CATHETERISATION SEQUENTIAL STEPS 18. Clean the area & cover it 19. Fix urobag below the level of the patient

15. NASO-GASTRIC TUBE INSERTION SEQUENTIAL STEPS 20. Thank the patient & throw the glove into yellow color bin

16. NASO-GASTRIC TUBE INSERTION SEQUENTIAL STEPS-RECAP 1. Introduce yourself ,Explain the procedure and take informed consent 2. Ask for a chaperone &Provide privacy 3. Don apron/ wash hands/ don gloves 4. Position patient 5. Put absorbent pad under patient 6. Clean genitalia properly 7. Apply sterile drape 8. Inject lignocaine jelly into urethra 9. Wait for 5 minutes 10 Place sterile receiver 11. Select appropriate catheter 12. Insert catheter gently into urethra 13. Fully insert the catheter until urine drains 14. Inflate balloon with sterile water 15. Attach Urobag 16. Pull catheter to conform self-retaining status 17. Replace prepuce if appropriate 18. Clean the area & cover it 19. Fix urobag below the level of the patient 20. Thank the patient and throw the glove into yellow color bin
