Hormonal Therapy IN cancer Mr Yogendra Mehta Lecturer, TU IOM BRNC

Hormonal therapy Hormonal therapy, or hormone therapy, is the use of hormones to treat cancer. A hormone is a substance produced by one tissue to influence the physiology of another tissue. Hormones stimulate some cancer cells, whereas they inhibit others. Depending on the characteristics unique to each cancer, hormones can be used to inhibit, or even kill cancer cells.

Hormonal therapy contd. Hormonal medications suppress the immune system and block normal hormones in hormone sensitive tumor. Hormonal medications change the hormonal balance and slow the growth rates of certain tumors

Hormonal medications Estrogen Diethylstilbestrol ( Stilphostrol ) Estramustine ( Emcyt ) Ethinyl estradiol ( estiny ) Polyestradiol ( estradurin ) Antiestrogen Anastrozole ( Arimidex ) Tamoxifen citrate ( Nolvadex ) Testolactone ( Teslac ) Letrozole ( Ferema ) Reloxifem ( Evista )

Hormonal medications and enzymes Androgen Fluoxymestrone ( Halotestin ) Testoterone Progestin: Medoxyprogesterone ( Depo-provera ) Megestrol acetate ( Megace ) Antiandrogen Bicalutamide ( Casodex ) Flutamid ( Eulexin ) Other hormonal antagonist and enzymes : Aminoglutethimide ( cytadren ) Asparaginase ( Elspar ) Leuptolide acetate ( Lupron ) Mitotane ( lysodren )

Side effects of hormonal therapy Anorexia Leukopenia Impaired pancreatic function ( Asparaginase ) Gynaecomasta Breast swealling Hot flashes Weight gain Hemorrhagic cystitis, hypouricemia , hypercholesterolemia, ( Mitotane ) Hypertension Thromboembolitic disorder

Side effects contd. Edema Sex characteristics alteration Electrolyte imbalance Edema, hypercalcemia , elevated cholesterol and triglyceride level ( Tamoxifen citrate) Tamoxifen citrate decreases the effects of estrogen Impotence and gynaecomastia in men (Diethylstilbestrol) Alter effects insulin, oral anticoagulants, hypoglycemic agent (Diethylstilbestrol)

Interventions Monitor vital signs Assess medications that the client is taking currently Monitor serum calcium level with androgens. Monitor for signs of alterations in sexual characteristics Monitor pancreatic function with ( asparaginase ) Encourage an oral intake of 2 to 3 L of fluids per day Monitor uric acid and cholesterol levels Monitor for signs of hemorrhagic cystitis
