Case Presentation on Ulcer Dr. Santoshi Dash, DNB Resident, General Surgery.

Patient Profile Hokato 35yr old, male Dimapur DOE: 09/01/2024

Chief Complaints: Chronic wound on front of left leg.

HOPI: Patient was apparently alright 1 month back, when he developed a wound on anterior aspect of left leg post trauma, associated with mild pain, aggravated while walking and relieved on rest. Patient is a known case of Type II Diabetes Mellitus on OHA.

LOCAL EXAMINATION INSPECTION: 12cm x 8cm, rectangular in shape, Single, Anterior aspect of left leg, 15cm distal to knee joint and 10cm proximal to ankle joint, Irregular margin with fibrosed edges, Healthy granulation tissue, Hypopigmented surrounding skin.

PALPATION: No local rise of temperature, non tender, Firm edges, Healthy granulation tissue with pin point hemorrhagic spots, Firm base, Side to side mobility present.

Regional lymph nodes- not palpable Femoral, popliteal, posterior tibial and dorsalis pedis artery pulsations felt in both lower limbs.

Summary: A 35year old, male, presented, with a single wound of 12cm x 8cm, rectangular in shape on anterior aspect of left leg with irregular margin, firm and fibrosed edges, healthy granulation tissue, hypopigmented surrounding skin.

Provisional Diagnosis: This is a case of chronic non healing ulcer. Differential Diagnosis: Diabetic Ulcer Trophic Ulcer Varicose Ulcer
