NEWER PERSPECTIVE IN COMMUNITY BASED REHABILITATION
EMBEDDED IN PRIMARY HEALTH CARE PEOPLE WITH DISABILITY (HANDICAP) EARLIER CONCEPTS OF REHABILITATION
Definition of Community based Rehabilitation CBR is "a strategy within general community development for rehabilitation, equalization of opportunities, and social inclusion of all people, implemented through the combined efforts of people with disabilities themselves, their families and communities, and the appropriate health, education, vocational, and social services" (ILO, UNESCO, & WHO, 2004). MULTISECTORAL APPROACH
MULTISECTORAL/MULTIDISCIPLINARY CLOSE CO-ORDINATION COLLABORATION CO-OPERATION BETWEEN GOVT/NON GOVT DECENTRALISATION OF RESPONSIBILITY, RESOURCES (5Ms)
GOAL &Objectives of CBR To promote a development that will allow the society to live a life of dignity. Rehabilitative services provided Elimination environmental barriers Providing compensation for disabled people Sensitization of society
Principles of CBR Solidarity Dignity
COMPONENTS OF CBR Creating a positive attitude towards people who need CBR Provision of functional rehabilitation services Provision of Education & Training Creating micro & macro income generation opportunities. Provision of care facilities Prevention of causes of disabilities Management, monitoring and evaluation
CBR MATRIX
THE SECTORS INVOLVED IN CBR Affected people Families of affected people Communities Governments (local, regional, national) Non governmental Organizations, local, regional and national) Medical professionals, allied health, educators, social scientists and others The private sector ( business and industry)
Advantage of CBR Programmes Home based Less expensive Existing community responses and resources Focus on quality rather than quantity Multiple approaches based on community needs
Limitations of CBR Programmes Different priorities. CBR programme should therefore be focusing on essential needs. Complex Organization- Multisectorial Low field activity- Educated workers rarely go to field and also find hard to communicate with low educated people . Low Social status to CBR worker- Frontline CBR is low profile job so less educated workers may influence quality of services provided Lack of community ownership- Breakdown of traditional social structure that contribute to several problems Expensive approach- as focus on quality
Good CBR Programme Build on widespread and existing resources of community Ideas and skills which are existing in minds of family members, community and affected members Has inbuilt community level programmes Part of national agenda in dealing with disability Coordination between various govt programmes Hospital to add CBR component to their outreach services Professional and political commitment
