STATUS OF PREVIOUS MEETING ACTION POINTS IMPLEMENTATION SN ACTION POINT RESPONSE 1 All providers to use client intake forms and have it signed by individual client before transferring data into the registers. Providers have been guided accordingly and being implemented. 2 All providers to complete documentation in VMMC registers Being implemented 3 All providers to use the screening tool Being implemented 4 Weekly VMMC report to be submitted by Saturday 16hrs Still a challenge to some providers 5 To be holding Data Weekly updates together with TB/HIV team Being implemented 6 Facilities not doing well providers should be contacted Done 7

7. ACTION POINTS STATUS IMPLEMENTATION CONT.. 8 All providers are encouraged to report adverse events to DHO whether minor, moderate and or severe within 24hrs. No report received yet. 9

8. ANNUAL PERFOMANCE AGAINST TARGET FY 25 Annual Target=2,538 Perfomance October 2024 to july 2025=2,355 Achievement in %=93% as of 11/08/25. JULY TO AUG TARGET=1270 CURRENT PERFOMANCE=675 ACHIEVEMENT IN %=53%

9. CHALLENGES Late weekly reporting by MC providers Other providers not reporting weekly. INTERVENTIONS Continue encouaraging providers to report on time weekly.
