INTRODUCTION Unless you can count it, weigh it or express it in a quantitative fashion, you have scarcely begun to think about the disease in a scientific fashion. - L ord Kelvin

ORAL HYGIENE INDEX [OHI] John C. Greene and Jack R. Vermillion developed the Oral Hygiene Index (OHI) in 1960. The OHI consists of 2 components :- Debris Index (DI) Calculus Index (CI)

Each of these index is in turn based on 12 numerical determinations representing the amount of debris or calculus found on the buccal and lingual surfaces of each of three segments of each dental arch. ORAL HYGIENE INDEX [OHI]

RULES OF ORAL HYGIENE INDEX 4. The tooth in a particular segment used for the calculation must have the greatest area covered by either debris or calculus. 1. Only fully erupted permanent teeth are scored. 3. From each segment one tooth is used for calculating the individual index, for that particular segment 2. Third molars and incompletely erupted teeth are not scored (because of the wide variations in heights of clinical crowns)

ORAL HYGIENE INDEX SIMPLIFIED [OHI-S] John C. Greene and Jack R. Vermillion developed the simplified Oral Hygiene Index (OHI-S) in 1964. The OHI-S differs from the original OHI in the number of the tooth surfaces scored (6 rather than 12), the method of selecting the surfaces to be scored, and the scores, which can be obtained. Only fully erupted permanent teeth are scored (A tooth is considered to be fully erupted when the occlusal or incisal surface has reached the occlusal plane). Natural teeth with full crown restorations and surfaces reduced in height by caries or trauma are not scored. An alternate tooth is then examined.

ORAL HYGIENE INDEX SIMPLIFIED [OHI-S] Section Of Tooth The 6 surfaces examined for the OHI-S are selected from 4 posterior and 2 + anterior teeth. - Facial surfaces of 16, 11, 26 and 31 -Lingual surfaces of 36 and 46

PATIENT HYGIENE PERFORMANCE[PHP] INDEX Introduced by Podshadley A.G. and Haley JV in 1968. Assessments are based on 6 index teeth. The extent of plaque and debris over a tooth surface was determined.

Apply a disclosing agent before scoring. Patient is asked to swish for 30 sec and then expectorate but not rinse. Examination is made by using a mouth mirror. Each of the 5 subdivisions is scored for presence of stained debris: 0= No debris(or questionable) 1= Debris definitely present. PROCEDURE

PHP index for an individual= (Sum of debris score/number of debris score) PROCEDURE

Excellent: 0 (no debris) Good: 0.1-1.7 Fair: 1.8-3.4 Poor: 3.5-5.0 SCORING CRITERIA

TURESKY - GILMORE- GLICKMAN MODIFICATION OF THE QUIGLEY - HEIN PLAQUE INDEX Quigley G. Hein. J in 1962, plaque measurement that focused on the gingival third of the tooth surface. They examined only the facial surfaces of the anterior teeth, using basic fuchsin mouthwash as a disclosing agent.. The Quigley-Hein plaque index was modified by Turesky S, Gilmore N.D and Glickman in 1970 Method: Labial, Buccal and lingual surfaces are assessed after using disclosing agent. INDEX SCORE=Total Score/ No of surfaces examined 0-1 = low  >2 = High

PLAQUE INDEX Silness and Loe in 1964 Assesses the thickness of plaque at the cervical margin of the tooth closest to the gums. All four surfaces are examined Distal,Mesial,Lingual,Buccal Despite being red, Mars is a cold place, not hot Mercury is the smallest planet in our Solar System Venus has a beautiful name, but it’s too hot Saturn is composed of hydrogen and helium

PLAQUE INDEX SCORE : 1 2 3 Despite being red, Mars is a cold place, not hot Mercury is the smallest planet in our Solar System Venus has a beautiful name, but it’s too hot Saturn is composed of hydrogen and helium Scoring Criteria CRITERIA : NO PLAQUE A film of plaque adhering to the free gingival margin and adjacent area of tooth Moderate accumulation of soft deposits within the gingival pocket, or the tooth and gingival margin Abundance of soft matter within the gingival pocket and/or on the tooth and gingival margin

PLAQUE INDEX Despite being red, Mars is a cold place, not hot Mercury is the smallest planet in our Solar System Venus has a beautiful name, but it’s too hot Saturn is composed of hydrogen and helium Calculation Plaque index for area : 0-3 for each surface. Plaque index for a tooth: Scores added and then divided by four. Plaque index for the individual: Indices for each of the teeth are added and then divided by the total number of teeth examined. Plaque index for group: All indices are taken and divided by number of individual

PLAQUE INDEX SCORE : 0.1-0.9 1.0-1.9 2.0-3.0 Despite being red, Mars is a cold place, not hot Mercury is the smallest planet in our Solar System Venus has a beautiful name, but it’s too hot Saturn is composed of hydrogen and helium INTERPRETATION RATING : EXCELLENT GOOD FAIR POOR
