Instructions to fill OMR Sheet of Answer Booklet
Do not fold the OMR sheet. Fill the details as follows; Degree- Basic BSc Nursing Exam- 1 st Year Subject- Write the subject Name Candidate Name- Write your name Regd No- Enter all seven digits correctly . Use only alphabets/ Numbers and darken the circles completely.
Fill the form neatly with dark black ball pen only. This form will be scanned by the computer. Do not enter any stray marks on this form. Fill the form in capital letters only. QP code- Enter QP code as mentioned on the Question paper/ hall ticket. Exam date- Enter date of exam in dd /mm/ yy format. Signature of the candidate- Affix your Signature.
After completion of exam, do not forget to tick the answered questions.
General Instructions
Do not write anything on the answer sheet other than answers. Write answers near the given question number only. Write answers neatly and legibly. Do not Use colour pens. Answer all the required number of questions. Under line headings.
No additional will be given. Write answers in points. Do not write paragraphs . Use pictures, flow charts where ever necessary. Anatomy and physiology and Nutrition and biochemistry need to write in separate booklets.
Adjust your time to answer part A an Part B in case of Anatomy and physiology/ Nutrition and biochemistry. Adjust your time to answer every question. Try finishing your exam atleast 15 mts before. Cross check your answers. Strike out or mark X in the empty pages.
All students must be in uniform with formal slippers/ No shoe. Carry your hall ticket/ ID card. Boys must be in formal hair cut and clean shaved beard. If not you will not be allowed into the exam hall. Do not carry pencil pouch/ mobile phones/ bags.
Do not carry chits. Mal- practice in any form is an offense. Serious action will be taken. Institute will not be responsible for such blunders.
Reach exam centre 1 hour before the commencement of exam. Relax and make yourself comfortable in your allotted seat. Read question paper twice and then start writing your exam. Cover your answers with question paper.
Do not turn towards others as the Web camera can capture......
Don’t skip your breakfast.
Carry RTPCR test report as per the instructions of the exam centre.
