PRIMARY CONCEPT: LEARNING The primary concept of lifelong learning means that persons have learning opportunities at all ages and in numerous contexts: CRADLE TO GRAVE All AREAS: at work, at home and through leisure activities, not just through formal channels such as school, Medical school and higher education. Extension of educational opportunities to people who have completed their education at school and higher education , in the presence of a demand for further education or training of any sort, also known as continuing education.

LIFELONG LEARNING During this era, lifelong education is a ART /SCIENCE OF EDUCATION often accomplished through From the professional point of view it includes postgraduate programs for those who want to improve their qualifications, bring their skills up to date or re-train for a new line of work. Contemporary lifelong learning or continuing education can take many different forms. Colleges and universities have instituted evening programs, extension work, and courses without credit, correspondence courses, and distance learning programs.

Lifelong learning for physicians: General principles Medical education in it s ideal form is one of the best examples of a lifelong learning process that begins in MBBS, extends into post graduate medical education , and continues throughout a physician's professional life . There has been a tremendous demand for lifelong learning for physicians in response to increasing and changing educational needs of practicing physicians , fostered by technical innovations , evolution of practice styles, and the reorganization of health care delivery . EMPHASIS WAS GIVEN ON DAY TO DAY ACTUAL PATIENT ENCOUNTERS AND REAL LIFE SOLUTIONS. Physicians are expected to be life-long learners because updated and effective patient care should be provided while medical and clinical knowledge and skills and social requirements for patient care are rapidly changing. In addition, qualified clinical competence needs long periods of training and each physician has to continually learn as long as he/she works as a professional.

A greater emphasis is being placed on measuring and improving the clinical and fiscal outcomes of medical care. The unprecedented rapid expansion of medical knowledge and the information gleaned from clinical trials and entering the era of evidence based healthcare with unprecedented access to information technology with an increasing focus on evidence-based patient choice. Evidence-based patient choice is defined as offering patients research based information and the opportunity to influence decisions about their treatment and care. Such evidence-based patient choice and care definitely contributed to increasing demand for continuing education and contributed to the evolution of the concept of lifelong learning for physicians .

IMPORTANCE OF LIFELONG LEARNING 1. Healthcare is becoming ever more challenging in its complexities by technical innovations, evolution of practice styles 2. patients and their families have expectations for the highest possible quality care 3. leadership in health care system requires knowledge and efficient competencies 4. learning is important for career advancement requirement.

DEFINITION Despite the emphasis placed on physicians' lifelong learning, no universally accepted definition has been proposed. Lifelong learning is a complex and multidimensional concept, : Lifelong learning is the development of human potential through a continuously supportive process which stimulates and empowers individuals to acquire all the knowledge , values, skills, and understanding they will require throughout their lifetimes and to apply them with confidence, creativity and enjoyment in all roles, circumstances and environments . Facets of this broad definition, such as human potential , supportive process, creativity, and enjoyment defy measurement

Quality medical education, both during the undergraduate curriculum and in the post-graduate phase include except general medical knowledge, Decision Making, Knowledge Management, Health Economics, Practice skill, Evidence Based Medicine. If physicians do clinical work using a scholarly way of thinking with critical analysis of their own competencies and improvement by reflection , they will become an excellent life-long learner

COMPONENTS LEARNING MOTIVATION (knowing the importance of learning new professional knowledge or skill) ATTENTION TO LEARNING OPPURTUNITIES (attending CME, Seminars, Medical conferences, reading journals) SKILL IN INFORMATION SEEKING ( using appropriate means)

SELF DIRECTED LEARNING Self directed learning (SDL) form of lifelong learning is the most effective approach for improving physician performance and patient care outcomes.

ADAPTIVE LEARNING 1. PLANNING: identify the knowledge gap 2. LEARNING: by utilising available resources 3. ASSESSING: Do self-assessment and take external feedbacks and determine the requirements to change/ improve the practice 3 feedback questions to assess: What & why is the learning objective? What activities to undertake for it? Use in practice and assess need of further reinforcement . 4. ADJUSTING: apply necessary change in practice.

MEDICAL JOURNALS Clinicians are expected to remain updated with the medical literature to maintain their clinical competence . Latest information on researches, newer diagnostic techniques, newer treatment modalities, further advancement in knowledge of cause of disease With over 2 million biomedical articles published annually and medical literature that has grown increasingly complex, physicians struggle to remain informed of the many new therapies and diagnostic tools that relate to their practices

CME Continuing education and improvement of medical practice has long been a tradition in the medical profession. instructor directed programs The program enables to learn about the latest development and advancements in the particular medical field

CPD/ WORKSHOPS CPD or Continuing Professional Education (CPE ) is the means by which members of professional associations maintain, improve and broaden their knowledge and skills and develop the personal qualities required in their professional lives. CPD is defined as the commitment to structured skills enhancement and personal or professional competence .

E-LEARNING Lifelong education is a pedagogy often accomplished through distance learning or e learning continuing education, or correspondence courses . The introduction of the Internet, e-learning, i.e . the use of Internet technologies that enhance knowledge and performance was associated with integrated e-learning into CME programs. Compared to conventional learning, e-learning has the advantage that participants can choose the place and time of education themselves. Within a clinical context , the effect of Internet-based CME programs is comparable to traditional approaches of CME

e-learning is becoming increasingly popular as a tool to aid teaching and learning in medical education. It has several advantages over traditional face-to-face teaching: It allows flexibility, enabling busy clinicians to choose the time and place for learning within their clinical duties. Learning can be timed at an individual's own pace and re-visited whenever necessary. E-learning packages can range from a simple online collection of resources to supplement traditional teaching to a fully web-based interactive course with all teaching materials, assessments and support provided online.

CREDIT SYSTEM A system of credits is developed to express the professional value of continuing medical education activities. Each activity is credited with a certain score, which can be awarded to the participating specialist. CREDIT HOURS remain an ethical obligation subject to the disciplinary authority of the profession itself in degree accreditation and registration renewal. CME has become a collective obligation of the profession; in order to promote and make it effective , CME should be available to all physicians.

Ideally , the plan for lifelong learning should be entirely voluntary and it should be driven by personal motivation of learner. Maintenance of professional competence remains an exercise of lifelong learning and an essential requirement for the evidence –based medical practice .
