What is a logbook in health training? Logbook is one of the assessment tool used to assess the Competence Based Curriculum Used for continuous assessment A logbook is a structured record keeping tool used to document and truck a trainees or health professional's practical experiences, activities, procedures and patient interactions.

What is a logbook in health training? It is systematic , organized record that serves to demonstrate the individual’s development of clinical skills, competences and professional growth Used to verify what a student has completed It is integral to assessing trainees progression in a program 12/04/2024 UAHEB 4

Purpose/importance of logbook Audit the trainee's clinical or practical work for the purpose of in-course assessment (SBA). Helps the trainers to keep track of skills learning and address any gaps Required for registration with the professional councils. Skills development; practice, refine and demonstrate clinical and technical skills in a controlled and supervised environment For Reflection and future reference by the trainee 4/12/2024 UAHEB

Importance of logbook Provides a summary of experiences a student is supposed to gain during clinical/practical placements. Competency assessment. Entries can be used to evaluate trainees ability to perform specific tasks Continuity of care: logbook experiences prepare trainees to freely intergrate into health care workforce. Feedback and mentoring, review of logbook entries can form a basis for guidance and recomendations . 4/12/2024

Importance of Logbook Cont ’ Encourages meeting real patients, or interacting with real situation which is an essential, exciting and anxiety provoking part of the medical profession. Quality Assurance, students exposed to a wide range of experiences, promoting high quality education Encourages students to approach supervisors and plan how to complete their logbooks. Should discourage them from running around for signatures. Improves patient safety Instils professionalism in trainees, teach accurate documentation, ethical practice 4/12/2024

The design of the logbook In order to improve students skills and knowledge and to aid learning of core competencies, minimum recommended skills/cases/competences are prescribed. Areas are chosen from a wide breadth of clinical/practical as prescribed in the curriculum which are core professional competences. The log book size depends on the program 4/12/2024 UAHEB

The design of the logbook The principle of doing many cases repetitively overdrives the principle of grading few cases. Eg carrying out 20 vital observation is better than carrying out 2 vital observations well. All stakeholders should sign in the respective areas as designed 4/12/2024

Grading performance This candidate forged the performance rating. SI - Means that the student worked independently but under supervision Grade A means that the trainee has performed the task independently and very competently Grade C means that the trainee has performed the task with assistance of a supervisor or competent colleague Thus a tick under: SO , the GD would be U SI, the GD would be either A or B 14

Assessment of Logbook Schools will identify a staff in charge of monitoring students logbooks (as required). Not every staff should be involved in signing log books for consistency purposes. UAHEB examiners of practical will inspect the signed pages of the logbook each semester. The school will provide the examiners with sample signatures of supervisors A comment will be made regarding the progress. 4/12/2024

Guidelines for the logbook Assessment tool The weight of scores are hinged upon the area of core competences expected. All clinical/practical areas in a logbook are covered in the assessment tool Conveyor Belt System is applied to the marking of logbooks where each examiners marks specific clinical/practical areas 4/12/2024

Common Irregularities in logbooks Students performing multiple tasks on the same day in different wards or departments. Some Trainees signing or giving remarks as supervisors in their own or other logbooks .

Irregularities in logbooks Supervisors signing and commenting before candidate performs the task or far later after Unnecessary stamping in logbooks with forged stamps in most cases Some students handed in logbooks when not duely signed by their principals and themselves. S ome students filling all sections on their own including signing and putting remarks for supervisors

Irregularities in logbooks Students not having varying performance rating e.g more SI than SO/SA especially on rare conditions Students having different dates of performing tasks and supervisors remarks or comments

Irregularities in logbooks Students leaving many blank spaces Performing tasks not appropriate at that particular level of training and /or health facility Students or supervisors using different handwritings Supervisors signing against skills they haven’t supervised e.g a Dr/ nurse signing for microbiology procedures Supervisors signing against tasks above their level

Irregularities in logbooks Overcrossing in the logbook and whitewashing Only one supervisor signing on all the tasks/skills performed in a specific module Inappropriate grading e.g giving A (means trainee has performed the task independently and very competently, against SO/SA Common

Examples of irregularities noted in the recent marking by UAHEB Watch and judge your self

Forgery by the supervisor One supervisor for most of the modules in the logbook. This supervisor signed more than six modules in the students logbook of a certain school 24

Irregularities in logbook This candidate forged the performance rating. Candidate was graded with A yet he/she observed ( SO ) SO , the Grade would be U SI, the Grade would be either A or B 25

Multiple forgeries in one module Candidate did many cases ( 14) on the same day of 9 th August 2022 S upervisors using block signatures and date instead of signing and commenting on each tasks 26

Self rating by the candidate Student putting supervisors remarks by him or herself – see the difference in handwriting of the supervisor’s name and the remarks written by the student. Block date – meaning he did many cases in one day Block signature of the supervisor 27

Lack of supervisors remarks This is evident of self rating; supervisor did not comment nor sign on the procedures. With no supervision, this candidate’s ability to perform neonatal resuscitation is questionable! 28

Wrong dates Dates of performing a task are not in chronological order he did the procedure on 26 th 09. 2022 and then as he is completing the section he reports 2/09/2023 Supervisor signs in May 2023 29

Mistakes in the logbook Using white wash to clear mistakes in a log book Patients number in chronological order Stamp obtained on 5 th /05/ 2023 before procedures were done on 23/05/2023 30

Falsification of signatures in log book It is taken as act of forgery.(a form of malpractice) It is punishable even when identified after training. Leads to cancelling of academic papers and Deregistration with allied health professionals council Forgery of the logbook is one of the malpractice 4/12/2024

Recommendations to stakeholders Ensure accurate and truthful record in your logbook. Use a logbook for self reflection and professional growth. Mind about record of competences acquired and not just competences expected Attend practical placements as scheduled by the school Be ready for any other form of logbook assessment
