Soal Puskesmas Kabun yang terletak dikabupaten Rokan Hulu, pada tahun 2024 memiliki ibu hamil sebanyak 537 dengan persentase reaktif HBsAg pada ibu hamil tahun lalu sebanyak 1,4 %. Puskesmas memiliki fasilitas untuk melakukan pemeriksaan HbV DNA dan dokter dan tim Puskesmas yang telah dilatih tentang pencegahan dan pengobatan Hepatitis B pada ibu hamil sampai pemantauan bayi 9-12 bulan . Tahun 2024 ini target pemeriksaan HbsAg pada ibu hamil 100%, sementara sisa logistic yang ada dipuskesmas Adalah sebagai berikut : RDT HbsAg : 50 HBV DNA : 0 catrige Vaksin HBIg : 2 syringe RDT Anti HBS : 0 Obat antivirus Hep B : 0 botol 2

Perhitungan permintaan logistik 3

a. Rdt hbsag Ibu hamil = {(% target bumil DDHB per tahun x jumlah ibu hamil x 1 tes ) + (10% stok cadangan )} Ibu hamil = {(100% x 362 x 1 test) + (10% x 362)} = {(537 + 54)} = 591 test = 24 kotak nb : isi dalam kemasan / kotak 25 test Pemantauan bayi 9-12 bulan = {( jumlah bayi lahir dari ibu HBsAg reaktifx1 tes ) + (10% stok cadangan )} ={(95 % x 1,4% x 362 x 1 test) + (10% stok ) = {(7+1} = 8 test 4

Total kebutuhan RDT HBsAg = RDT HBsAg pada ibu hamil + RDT HBsAg untuk pemantauan bayi 9-12 bulan - sisa stok Total kebutuhan RDT HBsAg = = 591 + 8 – 50 = 549 ( 22 kotak ) nb : isi dalam kemasan / kotak 25 test 5

HBV DNA atau RDT HbeAg = {( Perkiraan jumlah ibu hamil HBsAg reaktif * x 1 test) + (10% stok cadangan )} – sisa stok ={(1,4% x 537) + (10% stok )} – 0 ={(8 +1)} – 0 = 9 test = 1 kotak nb : isi dalam kemasan / kotak 25 test 6

Vaksin HBIg {( Perkiraan bayi lahir dari ibu HBsAg Reaktif * x 1 vial) + (10% stok cadangan )} – sisa stok ={(1,4% x 537) + (10% stok ) – 0 = (8 + 1 ) – 2 = 7 syringe = 9 syringe permintaan Nb : 2 syringe dalam 2 bln exp 7

Anti-HBs = {( jumlah bayi lahir dari ibu HBsAg reaktif x 1 tes ) + (10% stok cadangan )} - sisa stok ={(1,4 % x 537) + (10% stok ) – sisa stok = (8 + 1 ) – 0 = 9 tes = 1 kotak nb : isi dalam kemasan / kotak 10 test 8

Obat antivirus hepatitis B ={( Jumlah bumil yang diperiksa HBV DNA atau HBeAg x 15% x 4 botol ) + (10% stok cadangan )} – sisa stok ={(1,4% x 362) x 15% x 4 botol )+(10% stok )}– sisa stok = 5 + 0 – 0 = 5 botol = 8 botol ( digenapkan ) 9
