Anatomy of Perineum (female)

The perineal area is conducive to the growth of pathogenic organisms because it is warm, moist and not well ventilated. There are many orifices situated in this area Urinary meatus Vaginal orifice Anus Pathogenic organisms can enter into the body through these openings if cleanliness not maintained. Introduction

“ Perineal care is defined as cleaning the perineum from the cleanest to less clean area, the urethral orifice to the anal orifice." Definition

To prevent sepsis. To remove discharges and prevent bad odor. To relieve itching. To promote healing of stitches. To promote comfort. purposes

The most important principle for the perineal care is to clean the perineum from the cleanest to less clean area. Principle

Patients who are unable to do self care. Patients with genito -urinary tract infections. Patients with incontinence of urine and stool. Patients with excessive vaginal discharge. Patients with indwelling catheter. Post partum patients. Patients after surgery on the genito -urinary system. Patients with injury, ulcers or surgery on the perineal area. Indication for perineal care

Assess the condition of the perineal skin –any itching, irritation, ulcers edema, discharge. Assess need and frequency of perineal care. Check physicians order for any specific instructions. Assess patients ability for self care. Check the articles available in the patient’s unit. Preliminary assessment

Explain the procedure to the patient to gain his/her cooperation. Provide privacy by screens and drapers. Remove all articles that may interfere with the procedure like air cushion. Give extra pillow to raise the head. Roll the draw sheet to opposite side to prevent soiling when bedpan is placed under buttocks over drawsheet . Offer the bedpan. Untie the pads if any, observe the discharge, its color, odor, amount. Arrange the articles conveniently on bedside. Preparation of patient and environment

Apply the medicine and pad if necessary. Remove mackintosh. Change linen if soiled. Make patient comfortable. Offer patient hot drink. After care of the patient

Take out the bedpan, remove the content, clean and replace. Remove screen and tidy up the unit. Clean all articles with soap and water, disinfect them and replace back. Wash hands. Recording and reporting Record the procedure with date and time and observations made and report it. After care of articles
