Fibroid uterus Particulars of the patient:- Name: Mrs. x Age: 25 years Sex: Female Religion: Islam Marital status: Married Occupation: Housewife Address: Moghbazar Date of admission: 10.9.2021 Date of examination: 12.9.21

Chief Complaints:- Excessive per vaginal bleeding during menstruation for 1 year. Heaviness/ Lump in lower abdomen for 6 months.

  History Of present illness:- According to the statement of patient, she was a regularly menstruating women with average flow and duration 1 year back. Then she noticed excessive bleeding during menstruation which was progressively increased in amount and duration. Now her period persist for 8-10 days. She stated of pain during menstruation which was associated during passage of clot. she did not give any history of dysmenorrhoea , dysperunia and cold intolerance. She also complaints of lump in lower abdomen which was gradually increasing in nature and painless. With these above complaints she admitted in this hospital for further management.

Past history:- Medical history: - She is non-hypertensive, non diabetic and no other medical disorder. Surgical history: - Nothing significant Family history:- Nothing significant Personal history: - She is non smoker and non alcoholic Drug history:- She took tab. Normens for 3 months. Socio-economic history:- Belongs to middle socio economic condition.

  Menstrual history:- Age of menarche-13 years Cycle- Regular Period- 8-10 days Flow-Excessive Dysmenorrhoea - Absent LMP-02.11.2021

Obstetric history :- Married for 6 years Para-2(NVD)-0 ALC-5 years

General examination:- Body built-Average Cooperation-cooperative Anemia- Mildly anemic. Jaundice-Absent oedema- Absent Dehydration- Absent

Pulse-72 beat/min Blood pressure-110/70 mm of Hg Respiratiry rate- 16 breath/min Temp-Normal Thyroid gland- not enlarged Lymph node- not palpable Breast- Shows changes according to her age . Cardio-respiratory system- reveals normal to me.

Per abdominal examination:- On inspection- There is a lump in lower abdomen. Umbilicus centrally placed and inverted. No scar mark on abdomen. On palpation- There is a mass about 20 weeks size, occupying hypogastrium and part of umbilical region with well defying margin, irregular surface, firm in concistency , non tender, mobile from side to side but not above downwards. On percution - Dull over the mass.
