FIBROSIS

PATIENT’S NAME: Suresh
AGE: 42
SEX: Male
ADDRESS: Pilkhuwa
OCCUPATION: Farmer
OPD NO: 240925867
CHIEF COMPLAINT:
Patient complains of restricted mouth opening , burning
sensation and pain in his lower right back tooth region
since 1 month.
HISTORY OF PRESENT ILLNESS:
Patient was asymptomatic 1 month back then he
experienced burning sensation and restricted mouth
opening with no associated symptom of pus ,swelling and
fever. Pain is sharp shooting ,sudden ,continuous
,radiating ,aggrevateson chewing and relieves on taking
medicine.
DENTAL HISTORY:
Extraction 10-12 years back
MEDICAL HISTORY:
No relevant medical history
FAMILY HISTORY:
No relevant family history
PERSONAL HISTORY:
HABIT-History of tobacco chewing 2-3 times a day
since 25 years.
ORAL HYGIENE-Patient brushes once a day
EXTRAORAL EXAMINATION:
•On extraoralexamination the patient was of moderate built, had a normal gait.
•Face was apparently symmetrical.
•Lips are competent.
•Lymph nodes: No abnormality detected.
•TMJ : No abnormality detected.
DENTAL EXAMINATION:
•Class 1 caries irt35, 36, 37
•Class 2 caries irt26
•Grossly decayed irt16
•Missing irt11, 12, 15, 21, 22, 31, 32, 33, 41, 42, 43
•Grade 2 mobility irt47
•Stains++ Calculus+
SOFT TISSUE EXAMINATION:
•Tongue/ Lips : No abnormality detected
•Hard and Soft palate: No abnormality detected
•Floor of mouth: No abnormality detected
•Buccalmucosa: Fibrous bands that are blanched are seen on right buccal
mucosa extending from 1

molar region to 3

•Gingiva: Generalized recession
MUCOSAL EXAMINATION:
•O/I: Blanching is seen on right buccalmucosa extending from 1
st
molar to
3
rd
molar region with reduced mouth opening of 30 mm.
•O/P: Fibrous bands are palpable.
•S/O: OSMF Grade 2
