Introduction Interviewing a patient for collection of data related to medical history is called medication history interview It is a detailed, accurate and complete account of all prescribed and non-prescribed medications that a patient had taken or is currently taking. It provides information of patients’ allergic tendencies, adherence to pharmacological and nonpharmacological treatments, self-medication with complementary and alternative medicines. 2
Identifies patients’ needs Explores the patient’s perspective of illness and its treatment Preventing prescription errors and consequent risks to patients. Detecting drug-related pathology or changes in clinical signs that may be the result of drug therapy Collect all currently and recently prescribed drugs, previous adverse drug reactions and adherence to therapy for the better health care plan Assess patient medication compliance Assess the rationale for drug prescribed 3 Need
Demographic information such as age, height, weight, education, occupation, type of residence etc Dietary information Social habits such as tobacco, alcohol, illicit drug use Current and/or past prescription medication such as name, dose regime, reason for medication or stopping of medication, outcome of therapy etc. Current and/or past Non prescription medication 4 Data collected
Current and/or past complementary or alternative medication ADR or Allergies- drug name, dose regime, reason for medication, date of reaction, description of reaction, treatment for reaction Vaccination Adherence to past treatment courses Medicines found to be ineffective. 5
