2. defenition Perineal care is defined as clean the perineum from the cleanest to the less clean area the urethral orifice to the anal orifice

4. Indication Delivery Episiotomy Dilation and curettage Vaginal operation Genital/urinary tract infection Fecal incontinence Patient with indwelling catheter

5. Preliminary assessment Doctors order for any specific instruction Assess the conditions of the perineal skin itching irritation Assess the need and frequency of care Assess the self care ability of the patient

6. equipments A sterile tray curved artery forceps-1 thumb forceps-1 bowl with 9 cotton balls K basin-1

7. A clean tray Clean gloves-1 pair sterile gloves-1 Anti septic solution(Dettol/betadine) A jug with warm water Bed pan Disposable mask Mackintosh Safety pins-2 top sheet apron

For female patient Explain the procedure to the patient It reduces anxiety and increase cooperation Prepare equipments in and assemble on right side of the bed Systematic ways of functioning saves energy time material Provide privacy To enhance comfort to the patient Place the mackintosh under the patient hip To prevent soiling of bed Position the patient dorsal recumbent that knees flexed and legs apart To provide easy to assess the genitalia Make a diagonal fold with top sheet and drop the client Keeep client draped until procedure begins minimizes anxiety

Keep the bed pan under the clients buttocks and pour warm water over the perineum Rinsing is more effectively than wiping Wash hands wear the gloves Reduce the spread of germs Open the sterile perineal pack and get the assistance help to pour the solution To prevent cross infection With help of thump forceps take the cotton ball clamp in the curved artery forceps To make a confident care Start the cleaning from less contaminated area to more contaminated area to reduce infection rate Use only one cotton ball for each stroke to reduce infection rate
