PHYSICAL ACCOMPLISHMENT
KRA OBJECTIVES PHYSICAL TARGET ACTUAL ACCOMPLISHMENT PERCENTAGE OF ACCOMPLISHMENT 1. HEALTH PROGRAMS AND SERVICES 1. Conducted nursing and medical services like the provision and prevention; and remedial measures for simple and common ailments in schools of East District every month of at least 10 learners and teaching staff of the school. Conduct appropriate nursing service to at least 10 learners and teaching staff of 12 schools with different interventions like: RBS, Vital signs, Physical assessment, SPO2 Nursing care management was given to the learners, teachers and parents as well during school visitation. Offered nursing actions during seminars, trainings of teachers in the venues. 40.3% 2 . Served as the Food Safety Officer-like in the Division Office and inspected the school canteen for its safety, cleanliness and being compliant on the different demands asked by DepED and LGU. Having able to check the school canteens’ environment and mechanism of food preparation and selling . This was made by surprise inspection to the canteens bringing the monitoring tools and instructed them of the different rules based on DOs. Conducted a meeting with the operators in my district and instructed them of the proper ways of managing the canteen and the compliance of different DepED Orders and Local Gov’t’s rules. Drafted a monitoring tool to be used during inspection based on what I have learned from the training of Basic Food Safety and FSCO. 40%

Monitoring the schools in adhering the Child-Friendly School System (CFSS) based on my monitoring tool that I made during the time of making the DEDP. 12 schools have to be monitored and check their status of adhering the system. Some schools have not yet informed nor knew about it that it need to undergo series of seminars related to it. 40%

Determined the nutritional status of the SBFP recipients and non-recipients of the elementary and secondary schools of City East District. 12 schools have all the Consolidated Nutritional Status and City East District’s Consolidated Nutritional Status (combined schools) Consolidated the baseline nutritional status of the learners (both elementary and secondary) 50% 2. Assisted the Division SBFP Coordinator in the distribution of NFP to the recipient schools as well was to the schools of City East Districts. City East schools’ receipt of the NFP. All deliveries from the suppliers were all received by the schools as evidenced by the receipts. 46.7%

Established partnership with stakeholders on the Implementation of Oplan Kalusugan sa DepEd Programs like the: *City Nutrition Council *PDAO *CHO *DOH Partner with different institution to strengthen the implementation of OK sa DepED program and other Health and Wellness programs of DepED . Teamed up with several government istitutions that helped DepED in the success and realization of Deped’s PAP. School Health personnel also participated on their activities and also applied it to DEpED -Pagadian City. 36.7%

Worked with the Philippine Disability Affairs Office (PDAO), CSWD, and National Council for Disability Affairs (NCDA) in determining the learners and DepED employees who are primarily diagnosed to be Persons With Disability in all schools and DepED office; and subject for Formal/ Medical Diagnosis. Building up a working relationship and work hand-in-hand in PAPs of both groups that help the people to go on with their lives with the guidance and assistance of the institutions. Having able to determine the learners and DepED employees who are worthy to be registered in PDAO and DOH as Persons With Disabilities. This was done by: Online Seminar of PWD Coordinators Check-up activity: ILHON ug ILISTA Submission of their filled-up forms to PDAO Procurement of PWD IDs 45%

Assisted the dental team during the different dental activities done in the schools. Collaborated with the Dental Aide and Dentists in some dental activities and helped them during their essential activities like tooth extraction and toothbrushing drill. 40% 2. Technically assisted the activities of the SGOD like the following: Assessment of the new schools for the issuance of the government permit. Monitoring the conduct of the Division/Regional/National Exam. Evaluation of the schools during the Brigada Eskwela Evaluation of the school’s building structure. Conduct of Mental Health to the clientele. Assisted the SGOD Section of the different activities done in the whole year as per order. Helped the SGOD personnel and allowed myself to be one of the members of the monitoring team during the different activities together with the SGOD personnel. 43%

Acted as resource person and participate in the webinars, trainings, activities and in information dissemination on different health matters. Acted as resource person, attended some trainings and participated in the information dissemination on health matters several times in a year as evidenced by the COP, COA, Memorandum, documentations, MOV’s, and etc. Several trainings, online seminars have been attended related to Health matters and for the improvement of health services. Several times being invited as speakers in some gatherings of teachers, learners, stakeholders, etc. 50%

Challenges, Issues, Gaps (CIGs) and intervention
CHALLENGES/ISSUES/GAPS INTERVENTION The rampant selling of food that have been advised not to be sold within the school. Instructing them not to sell those food items and refer always to the DepED Order No. 13., s. 2017. Non-compliance of the canteen operators in the different pertinent documents as the canteens continue to operate without the basic documents needed. School Canteen coordinators are instructed to read, comprehend and apply the DepED Orders for food safety. Ignorance of the food to be sold and not to be sold in the canteens. School canteen inspection shall be done by the school nurses and the involvement of the PTA officials, Baranggay Local Government. More learners and employees that need to be medically assessed to qualify as PWD. Set a schedule, of at least 2 days, to gather those concerned and be checked by the medical officers, PDAO representative, ZDSMS representatives.
NEXT STEPS, WAYS FORWARD AND COMMITMENT
Monitoring of the school canteens (Jan-Feb 2024) Conduct a seminar on Food Safety to the Canteen Operators, School Canteen Coordinators in partnership with the CHO’s Sanitary Inspectors. (Feb. 2024) Check the canteen operators’ compliance of the different documents for the year 2024 (e.g. Health Certificates, Sanitary Certificates, Business Permits, CDA) Invite a COA representative during the seminar in the making of financial reports. 5. Daily checking of the canteen using the monitoring tool. 6 .Selection of the Best School Canteen for District Level (March-May)
Virtual meeting with all the PWD School Coordinators. Recognize how many are yet to be checked and their location. Draft a proposal called, ILHON UG LISTAHON and add-on the Budgetary requirements to conduct the medical check and determination of the PWDs. Report the cases to the CSWD, PDAO and DOH, later to PhilHealth and SPED.
