Proof of implementation – Rating 5 Proof of Contextualization- Rating 4 Proof of Adaptation- Rating 3 Action Plan for adaptation and implementation of learning programs- Rating 2 To get a Rating of 5, I have provided the template of Accomplishment Reports, just encode your details in the template and attach some pictures with captions about your program.

11. MOVs No. of MOVs OBJECTIVE 11 1 You only need 1 MOV for this objective, you don't need to prepare all that document, choose only 1. Report on the implementation/completion-Rating 5 Program Plan-Rating 4 Minutes of a consultative meeting-Rating 3 Communication Letter–Rating 2 To get a Rating of 5, you need to have a report of completion or implementation to any of the following programs. ( e.g.Brigada Eskwela , Gulayan sa Paaralan , School-based feeding program, National Reading Program, etc )

12. MOVs No. of MOVs OBJECTIVE 12 1 You only need 1 MOV for this objective, you don't need to prepare all that document, choose only 1. To get a Rating of 5, you need your copies of your coaching and mentoring sessions, if you have a colleague who is a master teacher, ask the copies of the PMCF she used when he/she observed you. A copy of FGDs minutes can also be used to get a rating of 5 . I have provided templates for FGD minutes about coaching and mentoring, all you need to do is to encode your details let your school head/master teacher sign it.

13. MOVs No. of MOVs OBJECTIVE 13 1 You only need 1 MOV for this objective, you don't need to prepare all that document, choose only 1. Proof of Participation-Rating 5 Minutes of Meeting-Rating 4 Accomplishment Report- Rating 3 To get a rating of 5, you have to secure a proof of participation in any of the following implementation of policies or procedures of DepEd listed below. *certificate of participation *pictures during implementation Some policies/procedure (School-based feeding program, National Reading Program (NRP) Quarterly PTA meetings, Brigada Eskwela , etc.)

14. MOVs No. of MOVs OBJECTIVE 14 1 You only need 1 MOV for this objective, you don't need to prepare all that document, choose only 1. To get a rating of 5, you have to secure evidences of feedbacks through chat, certification, or observation like PMCF or STAR observation, of your superiors or stakeholders about your good practices that uphold the dignity of teaching profession. Example: *recognition from the school/community about your good qualities as a teacher *Good remarks noted by your superior during observation *Chat/Feedbacks from your colleagues because of your contributions to finish a task
