Hiatal Hernia What is hiatus ? What is Hernia.

Oesophageal Hiatus

Hernia Abnormal protrusion of a viscus or its part through a normal or abnormal opening in the walls of its containing cavity. Hiatal Hernia is abnormal protrusion of abdominal viscera into posterior mediastinum through oesophageal hiatus .

Types of Hiatal Hernia Type I –Sliding Type II- Rolling / Paraoesophageal

Types of Hiatal Hernia

Types of Hiatal Hernia Type III- mixture of type I and II hiatal hernia. In this type, in addition to the para-esophageal hiatal hernia, the GEJ is not fixed but displaced above the diaphragm Type IV hiatal hernia is the herniation of other abdominal organs (spleen, colon, pancreas, etc.) through the esophageal hiatus into the posterior mediastinum

Etiology

Etiology Idiopathic Congenital Traumatic Infections /Infestation Autoimmune Neoplastic (Benign/Malignant) Degenerative obesity, pregnancy, and ascites .

Pathophysiology

Pathophysiology Burkitt et al suggest that the Western, fiber -depleted diet leads to a state of chronic constipation and straining during bowel movement, which could explain the higher incidence of this condition in Western countries.

Clinical Features

Demography

Demography Hiatal hernias are more common in Western countries. The frequency of hiatus hernia increases with age, from 10% in patients younger than 40 years to 70% in patients older than 70 years. More common in Females -?pregnancy

Symptoms

Symptoms Asymptomatic Reflux and its complications intermittent dysphagia , Bloating chest pain, Regurgitation, Cough gastrointestinal bleeding heartburn. Incarceration of rolling hiatal hernia-abrupt with a sudden onset of vomiting and pain

Physical Examination

Physical Examination Normal Certain conditions predispose to the development of hiatus hernia. These include obesity, pregnancy, and ascites .
