In this type, in addition to the para-esophageal hiatal hernia, the GEJ is not fixed but displaced above the diaphragm Type IV hiatal hernia is the herniation of other abdominal organs (spleen, colon, pancreas, etc.) through the esophageal hiatus into the posterior mediastinum
Pathophysiology Burkitt et al suggest that the Western, fiber -depleted diet leads to a state of chronic constipation and straining during bowel movement, which could explain the higher incidence of this condition in Western countries.

Physical Examination Normal Certain conditions predispose to the development of hiatus hernia. These include obesity, pregnancy, and ascites .
