FORMAT Some schools did not use the prescribed AIP template provided in DO #44, s.2015 Some schools simply copied the template without properly checking its entry. Statement of “output of the year” needed revision. “Output of the year” is stated as “Activities” Some SIPs missed some important parts such as List of PIA, Root Cause Analysis and Project Work Plan. Projects were not categorized according to Access, Quality, & Governance Time frame of activities to be conducted was not specified . The AIPs of the schools visited only reflected resources from MOOE. Other resources like from PTA, LGU, etc. are not indicated. Areas for Improvement

Objectives were stated in general format/style. Objectives were written exactly the same since 2 020 to 20 23 Objectives were not stated in behavioral terms. Targets in a certain year which were not achieved and were not carried over to the next year. Areas for Improvement

retooling on the crafting the esip III. PROJECTS Some projects crafted are not based on the PIA with “Very High Priority” rank. Some projects were crafted without valid basis. No baseline data to tell why there is a need to craft such projects. Inconsistencies of baseline data and target There are a number of projects, of which some of these projects can be lumped together. There was inconsistencies in terms of figures in the data being presented. Projects are implemented but no data presented to show progress. Some of the root causes of the PIA are not addressed in the implementation of the project. There are projects implemented based on the PIA, others have no basis at all. Areas for Improvement
