TEPID SPONGE BATH
DEFINITION
►The application of tepid water
to the body’s surface with the
use of a washcloth to provide
cooling measures to patient
with elevated temperature.
Purpose
►To reduce fever by dilating superficial
blood vessels, thus releasing heat and
lowering body temperature
►Improve circulation
►Relieve restlessness
►Assist in the elimination of toxic materials
from the body
►Provide comfort to the client
EQUIPMENTS
Making a bath mitt,
triangular method
Lay your
hand on the
washcloth;
Lay your hand
on the
washcloth; , fold the side
corners over
your hand
tuck the second
corner under the
cloth on the palm
side to secure the
mitt.
Making a bath mitt,
rectangular method
lay your hand on
the washcloth
and fold one side
over your hand
fold the
second side
over your hand
fold the top of the
cloth down and tuck it
under the folded side
against your palm to
secure the mitt.
Procedure and Rationale
Assessment:
►Check the doctor’s
order per institutional
policy
►Identify and explain
procedure to the
client
►Assess if bedding
needs to be changed
►Some pediatrician do not
allow TSB to be done on their
client because rapid
reduction in temperature
can lead to convulsion
►Proper explanation allays
fear and anxieties, making
the client more cooperative.
►To avoid unnecessary trips to
get linen
Planning:
►Give prescribe antipyretic 15-20 mi
before starting the TSB
►Wash hands
►Assemble equipment and bring it to
the bedside:
1.Pour water basin until it is ½ full
2.Pour additional water into the
pitcher
3.Prepare HWB and ice cap
4.Place everything in cart and bring
to bedside
►Close windows.
►Provide privacy
►For rapid reduction of fever
when combined
►Prevent the spread of
microorganism
►Ensure ease and systematic
performance of the
procedure
►Minimize spillage
►Water should be change as
necessary
►Prevent chilling
►Exposure of body part
Implementation:
►Offer bedpan or urinal
►Replace top sheet with
bath blanket
►Remove all clothing and
place on the chair. Move
client near you
►Place ice cap on
forehead.
►Ask the client to bend the
knees, place soles on the
client’s feet on hot water
bag. Remove HWB 3o
minutes after the
application.
►Place towel under the
head
►To avoid interruptions in the
procedure
►The bath blanket is thicker and is
more effective in preventing
chilling
►Sponge face and neck about three
times then dry. Use different surface
of the was cloth. DO NOT APPLY
ALCOHOL.
►Sponge the following body areas in
the following order:
a.Hands and arms
b.Chest
c.Legs and thigh
d.Back
Note: in case of shock, stop treatment
then apply external heat. Report
condition at once to clinical instructor
e. abdomen-just spread wet wash cloth
over the abdomen
►To avoid drying of the skin
►Key points in sponging:
►Fold washcloth
►Wring washcloth gently to retain
water which can be applied
generously to skin, avoid wetting
the floor
►Sponge each part about 6 times
with long even strokes
►Use fresh surface of the washcloth
each time
►Change water as often as needed
to maintain the desired
temperature.
►After each part has been
sponged. Pat dry
►Assist client with clothing
►Replace bath blanket with top
sheet
►Open windows
►Give cold drinks unless client
complaints of chilling
►Take temperature 3o minutes
after bath
►Remove the ice cap prn
►Do aftercare of the equipment
►Wash hands.
►To make client comfortable
►Fresh air will make client
comfortable and freely circulating
air promotes heat loss through
radiation
►Fever increases the body’s fluid
loss , so client needs fluid
replenishment.. If the client is
chilling give the warm drink
instead
►Determines effectiveness of the
procedure
►Client’s room must be left neat
and tidy
Documentation:
►Document and include the
following:
a.Time and duration of
treatment
b.Client’s reaction to
procedure
c.Liquids given
d.temperature
►Indicate whether
TSB has been
effective.
Special Considerations
►Do not apply sponge cloth over the chest.
►Observe the patient carefully during and after procedure.
►Avoid unnecessary exposure.
►Do not use frictions as it raise temperature.
►Give sponge for 20 min, discontinue in case of any
untoward reactions (shivering,cyanosis or if the pulse
becomes rapid or irregular).
►Offer liquids as desired by the patient.
►Provide diet which is high in calorie and easy to digest.
AFTER CARE
►Remove the sponge clothes from the axilla and groin. Discard it in
kidney tray
►Dry the body with bath towel
►Replace the gown and remove the bath blanket
►Observe for any symptoms of chill or any other abnormality
►If needed give him hot drinks
►Position the patient comfortably in the bed
►Replace the articles after cleaning
►Wash hands
►Record the procedure in the nurse’s record sheet and vital signs in
TPR sheet
