Topic: Process Of Hospitalization Objectives Define term admission, transfer and discharge. Discuss the procedure for admission, transfer and discharge. Identify Nursing responsibility during admission, transfer and discharge. Discuss Nurse Role in preparing patient and family for discharge. Discuss the normal reaction of patient being hospitalized. 3

Admission: Admission is the entry of patient into a hospital or ward for therapeutic or diagnostic purposes. Admission is defined as allowing a patient to stay in hospital for observation, investigation, treatment and care. 4

Types of Admission Emergency Admission Routine Admission 5

Emergency Admission Urgent admission to hospital involves a sudden health issue that needs patient to go to an emergency department is called emergency admission. 6

Routine Admission : In this patients are admitted for investigation, diagnostic, medical or surgical treatment . 7

Transfer With the changing condition of patient weather improved or more critical may require transfer either to another unit in the hospital or to another health care institution such as nursing home or rehabilitation hospital. 8

Discharge: Discharge from the hospital is the point at which the patient leaves the hospital either returns home or is transferred to another facility such as one for rehabilitation or to a nursing home . 9

Types of discharge: i ) Planned ii) LAMA (Leave Against Medical Advice) iii) Abscond iv) Referrals v) Transfer   10

Admission Procedure: Admission procedure involves : Prepare the patients room Before the patient’s arrival, make sure their room is ready Admission check list ready Pen or pencil Gown or pajamas 11

Cont… Portable scale thermometer Make sure there is adequate light and ventilation Sphygmomanometer or stethoscope Washbasin, emesis basin Soap or towels or lotion Bed pan or urinal 12

Greeting the patient When patient arrives at hospital a nurse must: Greet each patient in a friendly, cheerful manner. Introduce yourself take patient to the room. Explain facility policy on visitors, telephone use. Tell patient when meal times are Answer any question 13

Cont… Have the patient put personal articles and other small belongings in the drawer in the bed side stand Make a list of clothing and items the patient is keeping Valuable should be sent home with family member If not they should be inventoried and placed in the valuable envelop with name, date, room number and description of items Give to supervisor or take to safe . Assist patient into gown or pajamas 14

Assessment of Patient Assess the patient’s general physical conditions appearance and behavior. Observe for Loss of function Sign of weakness Any prosthesis Physical complaints the patient has 15

Cont… Record vital signs Ask about previous hospitalization, allergies, disease Record all the information and observation on the admission check list. Make the patient comfortable in their bed or in a chair. 16

Cont… If put to bed, raise side rails if needed. Make sure that patient can reach the signal cord and other needed items. If patient is unable to answer have family member help. 17

Transfer procedure Patient may be transferred from one room to another for several reasons. Sometimes it is at the patient’s request for a different type of room or a more compatible roommate. Medical staff may request it change in level of care i -e ICU to medical surgery. Sometimes the staff will transfer a patient closer to the nursing station where the patient can be observed more closely. 18

Cont… Make sure that patient’s belongings are transferred with them. Collect belongings and any equipment Check all areas of the room for articles that might be forgotten The nurse will collect the patient’s chart and medicines 19

Cont… Document date, time of transfer, reason for transfer, patient’s attitude towards the move. Introduce the patient to the personal care for him/her in the new room Orient patient to new room comfort 20

Discharge Procedure Discharge planning involves the entire health care team The patient, the family, medical staff, nursing staff, social worker and dietician all work together to coordinate the discharge The doctor plans the discharge with the patient and leaves a written order on the patient’s chart The nurse will then make necessary arrangements with other departments to prepare for the discharge 21

Written orders by the doctor include : Taking medications Exercise programs Changing dressings or bandages Injections and respiratory treatments 22

Cont… Any home health care When to follow up a doctor Any discharge instructions reviewed with the patient must also be put in a written form for the patient to take home 23

Nursing responsibilities during admission transfer and discharge On Admission: Nurse should deal every effort to be friendly and courteous with patient and family members Make proper observation of patient’s condition Nurse show efficiency and concern to the patient Demonstrate in the skills laboratory how to orient a new patient to the health care facility 24

Cont… Discuss concepts related to caring for the patient’s clothing and valuable items List factors related to caring for the client dehumanization. State how each might be avoided State the nursing consideration related to the admission of a client. 25

Cont… The patient is usually asked to put on pajamas or hospital gown, the nurse may need to help the patient change clothes Clothing should be inventoried along with other items the patient uses Jewelry and money kept in the room must be recorded 26

On Transfer: The patient’s transfer requires through preparation and careful documentation Nurse should explain transfer to the patient and family discuss the patient’s condition and plan of care with the staff of the receiving unit or facility, arrangements for transportation Documentation of the patient’s condition before and during transfer and adequate communication among nursing staff ensures continuity of care 27

Nurse role in preparing patient and family for discharge One of the most important duties nurses have, is to prepare patient and family for discharge Teaching patient about their conditions, medications, self care, strategies and the importance of follow up care can help patient maintain an optimum level of health and reduce their chances of readmission to the hospital 28

On discharge Nurse should teach the patient and family about the patient’s illness and its effects on his/her life style Provide instructions for home care Communicate dietary or activity instructions   29

Cont … Explain the purpose, adverse effects and scheduling of medication treatment Planning can including arranging transportation Follow up care may be necessary 30

Normal reaction of the patient being hospitalized Fear of the unknown Loss of identity Disorientation Separation anxiety Loneliness The patient is usually very concerned about health problems and the potential outcome of treatment 31

The nurse may help reduce the severity of common reactions Have a warm caring attitude and be courteous Show empathy Treat patients with respect Maintain their dignity Involve them in the plan of care Whenever possible adjust hospital routine to meet their desires 32
